I, the undersigned, in consideration of the services of Priestess Presence LLC and the Emerald Temple hereafter referred to as PP/ET, hereby agree to release and discharge Priestess Presence LLC, and The Emerald Temple on behalf of ourselves, our heirs, assigns, personal representatives, and estate as follows:

1. I understand and acknowledge that the retreat (and its activities) that I am about to voluntarily engage in as a participant and/or volunteer bears certain known risks and unanticipated risks, which could result in injury, death, illness or disease, physical or mental, or damage to myself, to my property, or to spectators or other third parties. The following describes some, but not all, of those risks: immersion in the White Springs, Temple Work, meditation, Anointing with essential oils, In addition, while at the retreat, participants may be subjected to a wide variety of weather, terrain, natural hazards, and physical activities which may require more than usual physical exertion and attention.

2. Being aware that this activity entails known and unknown risks of injury to myself and a risk of injury to spectators or other third parties as a result of my actions, I expressly agree, covenant and promise to accept and assume all responsibility and risk for injury, death, illness or disease, or damage to myself, to others, or to my property arising from my participation in this activity. My participation in this activity is purely voluntary, no one is forcing me to participate, and I elect to participate in spite of the risks.

3. I hereby voluntarily release, forever discharge, and agree to hold harmless and indemnify PP/ET and all others, persons, or entities from any liability, claims, demands, actions or rights of actions, which are related to, arise out of, or are in any way connected with my participation in this activity, including specifically but not limited to the negligent acts or omissions of PP/ET and all other persons or entities, for any and all injury, death, illness or disease, and damage to myself or to my property.

IN SIGNING THIS DOCUMENT, I FULLY RECOGNIZE THAT IF ANYONE IS HURT OR PROPERTY IS DAMAGED WHILE I AM ENGAGED IN THIS EVENT, I WILL HAVE NO RIGHTS TO MAKE A CLAIM OR FILE A LAWSUIT AGAINST PRIESTESS PRESENCE LL or THE EMERALD TEMPLE, EVEN IF THEY NEGLIGENTLY CAUSED THE BODILY INJURY OR PROPERTY DAMAGE.

4. Should it become necessary for PP/ET or someone on PP’s/ET’s behalf, to incur attorney’s fees and costs to enforce this agreement, or any portion thereof, I agree to pay PP’s/ET’s reasonable costs and attorney’s fees thereby expended, or for which liability is incurred.

5. I certify that I have sufficient health, accident, and liability insurance to cover any bodily injury or property damage I may incur while participating in this event and to cover bodily injury or property damage caused to a third party as a result of my participation in this event. If I have no such insurance, I certify that I am capable of personally paying for any and all such expenses or liability.

PHOTOGRAPH I TESTIMONIAL CONSENT AND RELEASE

I, the undersigned, of legal age, hereby grant, irrevocably and perpetually to Priestess Presence LLC, and Emerald Temple (hereby after called PP/ET), and their subsidiaries, affiliates, successors and assigns thereof, the following:

The right to photograph, video and record my name, voice, appearance, likeness, and/or written testimony along with any material furnished by me, in whole or in part, in any program provided by PP/ET or any affiliate, subsidiary or assign of PP/ET.

The right to publish, exhibit and distribute the use of my name, voice, appearance, testimonial and/or likeness along with any material furnished by me, in whole or in part, worldwide, for any commercial purpose, including but not limited to the advertising or solicitation of business, by any means of mass and/or electronic media, including but not limited to print, radio, television and promotional materials, events and/or marketing plans.

I hereby release and agree to indemnify and hold harmless PP/ET their subsidiaries, affiliates, successors, assigns, officers, directors, agents, owners, employees, thereof, from any and all claims, actions, causes of action, damages, expenses, court costs, attorney fees, liability damage or judgment brought forth as a result of my participation or testimonial materials furnished by me.

I hereby acknowledge that I have not been paid or otherwise compensated, nor will I be compensated, for my participation to record and publish my name, voice appearance, likeness, and/or written testimony, along with any material furnished by me, in whole or in part. I further certify that all material, whether verbal, written or exhibited by me, has not been scripted and represents my individual opinions and beliefs, and are true and correct to the best of my knowledge.

I confirm and agree not to take any photographs or video footage whilst inside the chambers of the White Springs, in keeping with their terms and agreements and respect of the spring and it's trustees.
